One (1) Prize Winner will be awarded with one (1) CoolSculpting procedure valued at $1400. Prize is not transferable. Cash value $0.01. Sponsor reserves the right, at its sole discretion, to substitute a prize (or portion thereof) with one of comparable or greater value for any prize listed in this Official Rules. All prizes are awarded “as is” with no warranty or guarantee, either express or implied, and Sponsor expressly disclaims the implied warranties of merchantability and fitness for a particular use. All prize details are at Sponsor’s sole discretion. ALL TAXES ARE THE SOLE RESPONSIBILITY OF THE WINNER. Sponsor may issue a Form 1099 to any prize winner that wins a prize valued at $600 or more.
MEDICAL QUALIFICATION: Winners will be required to have a consultation with Dr Elaine Cook at Advanced Skin Treatment Center, Elaine Cook MD, 2609 Wolflin Village, Amarillo Texas 79109 (“ASTC, Amarillo”) ensure that they are qualified candidates for the procedure. Procedure will be performed at ASTC, Amarillo. Winners may have to travel to ASTC, Amarillo to receive the procedure, depending on their location. Winners are responsible for all costs and expenses associated with the travel to ASTC, Amarillo. Winners will have six months to redeem prize once Dr Cook determines they are qualified candidates for the procedure. All decisions of Dr Cook are final and binding. If the Winner is not deemed a qualified candidate for CoolSculpting, the Prize will be deemed forfeited. The ideal CoolSculpting candidate maintains a healthy weight yet struggles with bulges in common areas that do not lend themselves to easy removal through exercise or diet. They are seeking spot reduction for specific areas of fat, but are not considering a surgical procedure.
